Who's eligible
Physicians, residents, fellows, and advanced practice providers (PAs, NPs, SLPs, audiologists) practicing in the U.S. with an active individual NPI. One tote per clinician while supplies last, whether you request it or a colleague gifts it. Once you sign up and answer the questions, you can also gift up to 3 in any 30 days and 6 in any 6 months, whether or not you get one for yourself.
What we do with your answers
Your answers are saved to Prosidio's CRM with your NPI as you enter them, so the right person can follow up if you ask. To save you typing, we look up your practice using public sources such as the NPI registry and practice websites. See our privacy policy.
Signing in
Use Google, a password, or a one-time email link. Your confirmed email is your login, so you can finish later or check on your tote. Email links work once and expire after 30 minutes. A gift's link lasts 30 days.
